What You Need To Know

If you or a family member ever experiences an accident or serious illness, you (and your bank account) will be glad you put some safety nets in place. Hospital indemnity, accident, and critical illness insurance are optional plans that pay cash benefits directly to you if you experience a covered event, regardless of what your medical plan pays.

Things to Consider

When deciding whether to enroll in voluntary supplemental health insurance, be sure to consider the following:

  • Cost per paycheck. The cost of coverage is based on whom you cover as well as other factors (e.g., age, tobacco status, coverage option). You’ll be able to see the cost per paycheck when you enroll.
  • Your and your family’s needs. Accidents and serious illnesses can have a significant impact on your family, even with comprehensive medical coverage. Does a serious health condition run in your family? Are you or an eligible family member frequently hospitalized? Would you need financial help to offset the cost of a serious health situation or accident? If you answered yes to any of these questions, having voluntary supplemental health insurance could give you peace of mind.
  • Other coverage. Consider how the various types of voluntary supplemental insurance could fit in with other coverage you have.
  • You’ll still need a medical plan to cover the majority of your medical costs. These voluntary supplemental plans are not substitutes for medical insurance.

What the Voluntary Plans Have in Common

Expenses can mount when you or a family member is ill or injured. Your medical plan will help cover your medical bills, but what about those extra expenses, like meal delivery, transportation, or help with household chores?

Having extra insurance can come in handy, since these plans pay you cash to use however you want.

Here’s what you can expect from your voluntary supplemental coverage, whether you elect the accident, critical illness, or hospital indemnity insurance plan:

  • You receive cash for eligible claims, paid directly to you. You can use it for whatever you want.
  • You pay 100% of the plan cost through after-tax paycheck deductions. That means you won’t pay taxes on the money you receive when you file a claim.
  • You can enroll only as a new hire, during Open Enrollment, or if you have a qualified life event change during the year.
  • You can take the plans with you if you leave Synopsys or retire.

Extra Wellness Benefits — Get Screened, Earn Money!

You and your covered dependents can earn a $50 annual wellness benefit by completing at least one eligible health screening, such as blood tests, a colonoscopy, or stress tests.

Accident Insurance

You may not be able to avoid accidents, but you can minimize the financial cost. Accident insurance pays you a benefit in the event you or your covered family members are injured in an accident. It’s not a replacement for medical coverage, but it can provide extra peace of mind by helping you pay for accident-related copays, deductibles, hospital charges, transportation fees, and lodging expenses.

  • You can select between two plan options: a lower-priced option offering less coverage and a higher-priced option offering more coverage.
  • The plan covers a wide array of accident-related injuries, including fractures, dislocations, and second- and third-degree burns, as well as accident-related medical tests.
  • You’re guaranteed coverage without a medical exam.
  • You can use the money you receive for anything you want.
  • Accident insurance may be a good choice for you if you or your kids are involved in sports where injuries are possible.

Critical Illness Insurance

If you have a serious health condition, critical illness coverage can help lighten the load. Critical illness insurance pays you a benefit if you or a covered family member is treated for a major medical event (such as a heart attack or stroke) or diagnosed with a critical illness (such as cancer or end-stage kidney disease). It’s not a replacement for medical coverage, but it can provide extra peace of mind by helping you pay for a deductible, coinsurance, or costs for any other services that may not be covered (e.g., long-term rehabilitation, home modification) by your medical plan.

  • When you enroll, you can also enroll your spouse or domestic partner and/or children.
  • You have two coverage options for both yourself and your spouse or domestic partner: $15,000 or $30,000.
  • You have two coverage options for your child(ren): $7,500 or $15,000.
  • You can use the benefit multiple times for certain reoccurring medical conditions, but a six-month waiting period between reoccurrences will apply.
  • You can use the money you receive for anything you want.

Hospital Indemnity Insurance

How prepared would you be to pay for an unexpected hospital bill? Hospital indemnity insurance pays you a benefit in the event you or a covered family member is hospitalized for a covered accident, illness, or childbirth. It’s not a replacement for medical coverage, but it can provide extra peace of mind by helping you pay for copays, deductibles, and other incidental charges that can add up during a hospital stay.

  • When you enroll, you can also enroll your spouse or domestic partner and/or children.
  • You can select between two plan options: a lower-priced option offering less coverage and a higher-priced option offering more coverage.
  • The plan pays a lump-sum benefit for hospital admission and confinement, ICU admission and confinement, inpatient drug and alcohol recovery treatment, and inpatient mental disorder treatment.
  • You’re guaranteed coverage without a medical exam.
  • You can use the money you receive for anything you want.

Submitting a Claim

Need to file a claim? Just follow these steps.

  1. Sign in to myuhcfp.com and click Claims Submission from the Claims menu. If it’s your first time signing in, click Register and enter the required information, including your Group ID and Group Name. If you need help, call customer service at (888) 299-2070.
  2. Enter the required information, which includes:
    • Your employer’s name and location
    • Your full name, date of birth, and Social Security number
    • Your complete address and phone number
    • Your marital status and number of dependents
    • The last day you worked, if applicable
    • Details of medical event
    • Your physician’s name, address, and phone number, as well as the date(s) of treatment
  3. Upload documentation. Review item #3 of How to File a Financial Protection Claim for examples of acceptable forms of documentation.
  4. After you submit your claim:
    • UnitedHealthcare will confirm your submitted claim by phone or letter within five business days.
    • If your claim is approved, your benefit payment will be mailed to you or direct deposited, depending on your payment preference.
    • If your claim is not approved, a claim specialist will reach out to you by phone and letter to suggest next steps.
